If you are interested in receiving a vaccine at a Community Vaccine Clinic hosted by BMC, you are also able to fill out this waitlist form. After you submit the form, BMC will call, email or text you to schedule an appointment. Due to supply, it might take several weeks until you are notified about an appointment. Submitting this form does not guarantee an appointment. If you are able to get an appointment through another channel, please get vaccinated through that channel, even if you have already filled out the form. Fill out the form below:
If you are eligible for a vaccine and are having trouble accessing one online, you are able to call the State's Hotline at 211.
